It is a very ancient custom of the Church that one is not admitted to baptism without a godparent, a member of the Christian community who will help one persevere in the faith and in their life as Christian.

To be eligible to be a godparent, one must be a registered member at a Catholic Church, must have completed their 16th year, must have made all sacraments including if married, must have been married in a Catholic Church or have obtained a dispensation to be married elsewhere.
To obtain the sponsor certificate for baptism, call the church where you are a registered member and ask for a "Sponsor Certificate for Baptism".
